CANCELLATION POLICY

Novum Skin (the “Business”) schedules appointments exclusively for each client and incurs losses when appointments are canceled without sufficient notice or when clients fail to appear for scheduled services. In consideration of the Business reserving appointment time on my behalf, I agree to the terms and conditions set forth in this Cancellation Policy Agreement (“Agreement”). I acknowledge and agree that all appointments require a minimum of twenty-four (24) hours’ notice to cancel or reschedule. If I cancel or reschedule my appointment with twenty-four (24) hours’ notice or less, I authorize the Business to charge a cancellation fee equal to seventy-five percent (75%) of the total scheduled service(s) to the payment method maintained on file. I further acknowledge and agree that if I fail to appear for my scheduled appointment without prior notice (a “no-show”), or if I arrive more than fifteen (15) minutes after my scheduled appointment time and the Business determines that there is insufficient time to safely perform the scheduled service(s), such appointment may be deemed a no-show. In such event, I authorize the Business to charge a fee equal to seventy-five percent (75%) of the total scheduled service(s) to the payment method maintained on file. I understand that appointment times are reserved exclusively for me and that late cancellations and no-shows prevent the Business from accommodating other clients. I acknowledge that the cancellation fee represents a reasonable estimate of the losses incurred by the Business and is not intended to serve as a penalty. I authorize the Business to securely retain my payment information for the purpose of collecting fees due under this Agreement. I understand that failure to pay any applicable cancellation or no-show fee may result in the refusal of future booking privileges until the outstanding balance has been satisfied. The Business reserves the right to require prepayment or deposits for future appointments at its sole discretion. The Business reserves the right, in its sole discretion, to waive or reduce any cancellation or no-show fee in the event of a documented emergency or other extenuating circumstances. Any such waiver shall not constitute a waiver of this Agreement for any future appointment. I represent and warrant that I am at least eighteen (18) years of age or am the parent or legal guardian of the individual receiving services and have full authority to enter into this Agreement. I acknowledge that I have read, understand, and voluntarily agree to be bound by the terms of this Agreement. This Agreement constitutes the entire agreement between the parties with respect to the subject matter contained herein and supersedes all prior and contemporaneous communications or understandings relating thereto. This Agreement shall be governed by and construed in accordance with the laws of the State of Colorado.
